Entry into wellness programs

This is determined initially by your BMI and/or body fat percentage. Our minimum entry requirements are BMI >30 with no co-morbid medical conditions or a BMI >25 with at least 1 co-morbid medical condition or a body fat percentage of >/=32% in women or >/=25% in men.


Payment Policies

Our programs are fee for service. We accept Visa, MasterCard, American Express, Discover card. We do not accept checks. We also have a great no credit check installment plan with PayPal along with 2 excellent patient financing programs for all credit situations to meet your needs. Some of you may have healthcare spending accounts which can be used for our services as well.

Payment for services is made on your program enrollment day and we do provide superbills that you can submit to your insurance carrier so that you can request reimbursement for services. The amount of reimbursement will depend on your insurance carrier. We are out-of-network for all insurance carriers.


Refunds

Once your program is purchased there are no refunds.

If a client wishes to change from one program to another prior to the start of his/her program this is acceptable. If the new program is more than the previous program the additional fees will need to be remitted prior to the change becoming effective. If the new program is less than the old program then this amount can be used for additional services such as personal training, dietitian visits, etc. at our center or used toward a family member’d/friend’s weight loss program.

Saturday & Sunday by appointment

Forms

New Client Evaluation/Assessment form will be provided to each client at his/her initial consultation after it’s determined which program is the best fit for the client.


Labs & EKGs

Each client may obtain the required labs and EKG independently and then fax the results to our office at 909-354-3173. If a client chooses this option the results will need to be sent at least 10 days in advance of his/her appointment with the physician.


Use of fitness facilities

As a client of Body & Mind Medical Weight Loss Center, PC you may use the fitness facilities during business hours. There may be personal training sessions ongoing in the fitness area however we strive to maintain a respectful, quiet, comfortable environment for all of our clients. If there is equipment that you would like to use but it’s being used by another client, please let staff know and we can assist you with another piece of equipment or another exercise until the equipment desired is free.

Water is provided for each client (please bring your own bottle) as well as TV. We ask that if you bring your own MP3 device, tablet, etc. that you respect others and keep your music’s volume to an acceptable level.

We ask that children not be in the fitness area unless participating in a scheduled personal training session. This is due to safety issues.


Access to providers

Our providers participate in a rotating call schedule so that if you are struggling with cravings, emotional eating triggers, food shopping decisions, or need someone to talk to after-hours you will have a schedule listing what provider is on call for that evening. If it’s a medical or psychiatric emergency we ask that you call 911 or go to the nearest emergency room.
